Version 1.5 of Deluxe Folder Icon Creator is designed to be very stable, much more so than previous versions. You should no longer have problems obtaining file icons, even when reading them from floppy disks. This is because I replaced my icon getting routines with GetFileIcon v1.1*.
However, if the folder that you are making a custom icon for already contains a custom icon, your new icon still will not appear until you restart. As an alternative, you may (from the Finder) Get Info for this file, and paste your new icon onto the folder. I hope to have a solution to this problem soon.
If any other problems arise, increasing the application窶冱 memory allocation may help. Otherwise, please contact the me, providing any information given in dialog boxes, as well as any other relevant information.
